Reference Letter
To obtain a reference letter please complete the following:
1. Click on the form to the left, complete the form making sure to digitally sign it. Print off a copy and bring it into the Police Department or email a copy to cbrown@lonepeakpolice.com with a copy of your Drivers License, Passport, or any Utah State issued identification.document.
2. When the request is complete you will be contacted for payment and information on receiving the document.
(The $10.00 fee will include 2 completed and notarized copies of the document)
